Shift: Day Shift (Shift 1)
Hours: 08:00 – 16:30 (30-minute meal break)
The Occupational Therapist will evaluate, plan, and deliver rehabilitative services to patients in a skilled nursing or rehabilitation setting. The role involves helping patients improve functional abilities, mobility, and independence in daily living activities.
Key ResponsibilitiesConduct initial assessments and develop individualized treatment plans.
Provide therapeutic interventions to improve functional performance and ADLs.
Document evaluations, progress notes, and treatment outcomes accurately.
Collaborate with interdisciplinary team members (PT, SLP, nursing, social services).
Train and guide support staff and assistants as needed.
Ensure compliance with facility, state, and federal guidelines.
Participate in care planning meetings and family conferences when needed.
Adjust therapy plans based on patient progress and clinical observations.
Maintain a safe, supportive environment for all patients.
Work a flexible 7-day rotating schedule as required.
Active state license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA) .
Experience in SNF/LTC or rehab settings preferred.
Ability to start immediately due to urgent need.
Strong documentation and clinical reasoning skills.
